Dear Zilverenkruis Team,
I just have been calling for 5 minutes, my credit run out and I couldn't speak to anyone. Is there any way I can write an E-mail or get in touch somehow?
I need a written confirmantion that I am insured at Zilverenkruis from April until the September 2016.
Could you send a document to me with that confirmation?

You could try the chatservice. Choose " service and contact and type on chat.

the option that Feina gave you is a good one. It's even better when you chat with us from out the digital area Mijn Zilveren Kruis. In that case we can see that you're logged on and give you more detailed information.
A second option is to send a request to firstname.lastname@example.org. This is the email address of the foreign department and they can send you the form as well.
